We get it. There is a serious lack of knowledgeable, accessible perimenopause and menopause care in the U.S. Women are scrambling - searching for someone who will listen, validate their concerns, and help them find themselves amidst a swirl of hormonal changes.

That’s exactly why we created the Swan Peri/Menopause Care Collective—a supportive, empowering space where you don’t have to figure it all out alone. And the best part? We’re right in your backyard.

Why Us? And Why “Swan”?

We are nurse practitioners and women’s health experts with over 30 years of combined experience. As integrative health practitioners, we blend conventional and natural approaches to meet you where you are, helping you find and address the root cause of your symptoms. We’re passionate about helping women live their best lives - and it all starts with better care.

And as for the name? Well, our founder, Amanda Swan, just happens to have a pretty cool (and fitting!) last name (thanks, Dad). But beyond that, swans symbolize grace, transformation, and resilience—just like you. Perimenopause and menopause aren’t about fading away; they’re about stepping into a new chapter with strength, wisdom, and support. And that’s exactly what we’re here for.

Jen Loos, Worthy of Wellness Physical Therapy

Jennifer Loos is a compassionate, experienced physical therapist with a Doctorate from Northeastern University and over 18 years of experience in the field. She’s passionate about taking a personal, thorough approach to providing care that’s as unique as each person she helps, focusing on lasting recovery and well-being.

Her own experiences with physically challenging pregnancies and postpartum pain led her to specialize in women’s and pelvic health. Since 2018, she has pursued specialized training in women’s and pelvic health and remains an active member of the Pelvic Global Academy, ensuring she stays current on the latest developments in the field.

In 2021, Jennifer opened Worthy of Wellness Physical Therapy to focus exclusively on women's and pelvic health. She recognizes the complexity of the human experience across a woman's lifespan. She combines her clinical expertise and thorough evaluation skills with a passion for empowering and educating her clients to create a personalized healing experience. Her approach? A thoughtful blend of hands-on techniques, customized movement strategies, and holistic care that addresses her clients’ personal goals while maximizing function and quality of life.

Jennifer is here to educate, break down barriers, and bust myths. She provides a private and non-judgmental space for open dialogue and healing. So, bring on the tears, the laughs, the silliness, and the brutal honesty – Jennifer is ready!

Anne Koneru, PMHNP-BC

Anne completed her training at Johns Hopkins University, and is a board-certified Psychiatric Mental Health Nurse Practitioner (PMHNP-BC). She also has over a decade of experience working as a family nurse practitioner in primary care. She provides holistic care with evidence-based medication management.

You and your experiences are unique. Anne seeks to identify the root causes of your concerns and will work with you to create lasting and transformational improvements in your well-being.

One of her favorite quotes from Jon Kabat-Zinn is, "You can't stop the waves, but you can learn to surf." Life presents all of us with challenges, but you don't have to face them alone. Kim Boldon, Dermatological APRN

Kimberly Boldon is a Board-Certified Nurse Practitioner. She received both her Bachelor’s and Master’s degrees from Southern Connecticut State University, graduating Magna Cum Laude and with Honors. She is a member of Sigma Theta Tau, as well as the American Association of Nurse Practitioners. Upon graduating from Southern, she continued her training in Dermatology alongside several dermatologists, most notably – renown dermatologist Dr. Ronald Savin.

Treating medical and cosmetic patients in both the pediatric and adult populations- makes her skills versatile and all-encompassing to treat your dermatologic needs. Her true passion is medical missionary work. Kimberly has volunteered in Mozambique and Haiti. Both her and her husband travel to Ecuador annually as part of a team that provides medical care and surgery to individuals free of charge.

Kimberly resides in Cheshire with her husband, Keith Boldon, and their twin daughters, Kya and Kennedy.
